Augmentation adds volume with a silicone implant placed either under the breast tissue or under the muscle. A lift removes excess skin and repositions the nipple to change where the breast sits. A reduction removes tissue as well as skin to reduce size and weight.
Which one you need is decided by what has actually changed. If volume has gone but position is fine, an implant answers it. If position has dropped, an implant alone makes the breast heavier rather than higher, and a lift is the honest answer even though it is the longer operation and the one with more scarring.
Sizing is done against your chest width, your tissue cover and your skin quality, not from a cup size. The same implant reads completely differently on two different frames, which is why sizers are tried at consultation rather than chosen from a chart.
