It is done under general anaesthesia at a licensed surgical facility, usually in two to three hours, either through a closed approach with all incisions hidden inside the nostrils or an open approach with one small incision across the column between them.
The work itself is millimetric. Reducing a hump, narrowing a bridge, rotating a tip or straightening a septum are separate manoeuvres, and most noses need a combination of them rather than one. Cartilage is often repositioned or grafted rather than simply removed, because a nose that has been over-reduced is far harder to correct later than one that was left conservative.
It is also the slowest procedure in aesthetics to settle. The splint comes off in a week and the bruising fades in two, but the tip keeps refining for about a year. Anyone judging the result at six weeks is judging swelling.
